This is a great website. I love having all of this information in advance. I look forward to getting a complete picture that I can share with my list of doctors. It helps me to understand my health issues better. It is important at my age (76) acyually at any age, especially if you have multiple health problems.

I agree - forms to fill out ahead of time would be really nice.

I agree with the prior comment, it will be helpfull if we can print and pre-fill out the forms before the appointment date.

What I think will be helpful is to have whatever form(s) required online, they could be printed/completed and already filled upon arrival which helps speed up procedure. Questions may be addressed at time of appt.
