You can schedule a screening and obtain customer assistance right here on our website. Of course, if you cannot find what you’re looking for, our customer service representatives are happy to assist you.
Making an appointment
It’s easy to find a screening event in your area and schedule a screening online. You can pre-register online for up to 60 days from today’s date. For screenings beyond 60 days in advance, please call to check availability.
You may also schedule screenings for a total of 2 people. Please have their personal contact information and dates of birth handy. Remember that pre-payment is required, so have your credit card or check ready.
Doctors often recommend Life Line Screening, but you are not required to have a doctor’s referral to schedule a screening in most states. However, if a doctor’s order is required in your state, a Life Line Screening board-certified physician will provide it for you at no additional cost.
Payment options
We accept MasterCard, Visa, American Express, or Discover. For security reasons all credit card payments will be processed at the time the appointment is scheduled. There are no cancellation fees.
You may pay with a credit card or check when you schedule your screening by phone or online. If you need to make a payment on a screening you have already scheduled, please call toll free 1-800-449-2350 and a customer service representative will be happy to help you. We do not accept cash at screening locations.
If you would like to redeem a gift certificate, please provide the number on it when you call to schedule your screening. Also, please bring the gift certificate with you

To find out about screenings that have recently been scheduled in your area, please call toll free 1-800-449-2350 and request to be added to our call-back list. You can also sign up to receive our free e-mail newsletter that includes screening locations in your community.
Change or cancel your appointment
There are no fees to change your appointment. Please call toll free 1-800-449-2350 and a customer service representative will be happy to help you reschedule your screening.
If you need to cancel your screening: A full refund is issued if you call to cancel at least 2 days prior to your appointment. If less than 2 days notice is given, we will issue a Gift Card for the full amount to be used by you, or anyone you choose for future screening services.
Receiving Results
You can view your results online through our e-Results email program within 21 days of your appointment, free of charge. You will receive an email notification when your e-Results are ready to view. You can also choose to have your results mailed to you within 21 days. There is a fee of $3 for printing and shipping. However, if we find a condition that requires immediate attention, we will notify you on the day of your screening. If you take a finger-stick blood screening, those results will be available in 10 minutes
Refunds

If you paid by check or money order, you should receive a check in the mail approximately 1 week after you speak with the customer service representative. If you paid by credit card or debit card, you should see a full refund directly to your account within 1-2 business days.
Request copies of screening results
To request an additional copy of your results, please send us your name, address, phone number, and date of screening along with a personal check or money order for $15.00 made out to Life Line Screening. Mail these to:
