Eligibility

All of the following criteria apply to be eligible for the free screening giveaway:

1. Two people schedule two screening appointments online or by phone together for the same event from February 5 through February 14, 2010.
2. Screening attendance between February 5-February 14, 2010 is not required.  The screening event could take place at a future date.
3. The appointment was scheduled by calling the phone number 1-800-909-1041 or by signing up online through the Valentine Day Giveaway page.
4. No purchase is necessary to enter. If you'd prefer to enter the Valentine's Day giveaway without scheduling an appointment, visit our Contact Us page. Submit your contact information and mention Valentine's Day Giveaway in the comments field.  We will enter you in the drawing.

 

Winner Selection

1. All eligible entrants will automatically be entered in the drawing
2. On or about February 16, 2010, Life Line Screening will randomly select seven pairs of entrants from all eligible entries received and give a total of 14 free screening packages.
3. Odds of winning depend upon the total number of eligible entries received.
4. The vascular screening package name is the Stroke, Vascular Disease and Heart Rhythm Package and has a retail value of $139.
5. Winner will be notified via phone, e-mail, or postal mail on or about February 16, 2010. 
6. Winner's List:  For winner's name, send a separate self-addressed, stamped envelope to Life Line Screening Valentine's Day Giveaway. E-Commerce Manager, 6150 Oak Tree Blvd, Independence, OH 44131 by March 15, 2010.

2. Select an event to view package options
3. Select the  Add Guest button on the packages page
4. Confirm the appointment times for you and your guest
5. Enter your contact and payment information

How to Schedule in Pairs by Phone

1. Call the 1-800-909-1041.
2. Register for you and someone else at the same time.

Vascular Screening Package

The free vascular screening package includes the following tests:

1. Carotid artery disease
2. Peripheral arterial disease
3. Atrial fibrillation
4. Abdominal aortic aneurysm
