Giveaway Rules

Eligibility

All of the following criteria apply to be eligible for the free screening giveaway:

1. Two people must reserve screening appointments together, online or by phone, between October 1 and October 31, 2010.
2. Screening attendance in October is not required.  The screening event could take place at a date beyond October 31, 2010.
3. No purchase is necessary to enter. If you'd prefer to enter the October Pairs Special $500 Giveaway without scheduling an appointment, visit our Contact Us page. Submit your contact information and mention October Pairs Special in the comments field.  We will enter you in the drawing.

Winner Selection

1. All eligible entrants will automatically be entered in the drawing
2. On or about December 1, 2010, Life Line Screening will randomly select five pairs of entrants from all eligible entries received and give a total of 10 free screening packages.
3. Odds of winning depend upon the total number of eligible entries received.
4. The vascular screening package name is the Stroke, Vascular Disease and Heart Rhythm Package and has a retail value of $139.
5. Winners will be notified via phone, e-mail, or postal mail on or about December 1, 2010.
