As a Nurse Practitioners on a medical outreach team, you will play a dynamic role. You will have autonomy and flexibility to specialize in preventive healthcare while focusing on what motivates you most – affecting the health of others. With the support of a Medical Assistant and a Collaborating Physician as needed, you will be providing health and wellness services to seniors to help them improve their overall quality of life while encouraging medically appropriate follow-up care with their physician.

  • Average salary $110K-$120K

Career Roles & Responsibilities

  • Administer Medicare Annual Wellness Visit to Medicare recipients
  • Administer Preventive Physical Exam, health education, and select vaccinations
  • When needed, coordinate patient’s care and confer with physicians regarding follow-up
  • Develop personalized prevention plan for patients, including follow-up preventive screenings
  • When needed, provide Advanced Care Planning

Career Requirements

  • Available 4-5 days/week Monday-Saturday
  • NPI #
  • Active State License
  • ANCC or AANP board certified required
  • Valid driver’s license and a clear driving record
  • Flexibility to commute in a company vehicle or by personal vehicle to a new location each day. Commute varies from 20 minutes to 4 hours.
  • Team environment to set up equipment and supplies
  • Capable of multi-tasking in a fast-paced environment
  • Ability to work autonomously and adapt to changing environments
  • Strong Computer Proficiency
