  • Available to San Bernardino County Employees, First Responders, or residents of the Second District. (Includes Fontana, Rancho Cucamonga, Upland, Mt. Baldy, San Antonio Heights, and the Foothills)
  • Availability is limited. Vouchers are distributed on a first-come, first-served basis
  • Screenings must be scheduled through Life Line Screening using a valid voucher code
